Dear Parent/Guardian of Boerne ISD Athletes:

The Boerne Independent School District Athletic Department would like to thank you for taking part in the electronic submission of athletic participation forms. All student athletes entering grades 7-12 are required to submit these forms electronically each year prior to participation in athletics.

The only required form that cannot be submitted electronically is the UIL Pre-participation Physical Evaluation-Medical History Form that is completed by the physician, a Physician Assistant licensed by a State Board of Physician Assistant Examiners, a Registered Nurse recognized as an Advanced Practice Nurse by the Board of Nurse Examiners, or a Doctor of Chiropractic. Examination forms signed by any other health care practitioner, will not be accepted.

You may print the UIL Pre-participation Physical Evaluation-Medical History Form by clicking on the “Download and Print” tab at the top of this page. The physical & medical history form must be completed and signed by a parent, the student, and a physician. The physical must be dated after April 1st of the upcoming school year.

Please submit the physical form to the middle school coaches or the high school Athletic Trainer (BHS – Megan Patton for CHS – Terry Gault). It is the recommendation of the Boerne ISD Athletic Department that you make a copy of the physical for your files.

Please note the following:

Students must have required paperwork on file in order to participate in athletic activities including practices, workouts, tryouts and athletic contests.

Parents, you must fill out each form.

* To access the online forms hold your cursor over the “Electronic Participation Forms” tab. You will get a drop down list of the forms.

*Click on the form name and fill out the information requested.

* To sign the document click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over click on the refresh icon next to the signature box.

* Once you have filled out all the information on each page, you will have the opportunity to print the document.

* You will receive a confirmation email once the staff has reviewed the document.

* Note for physicals: Students are required to use the Pre-participation - Physical Examination Form provided. NO OTHER Physical Examination Form will be accepted. A new physical exam must be completed prior to each school calendar year. The form is accessible from the Download and Print tab.
