If your child has been identified as having a disability and has special dietary needs, changes can be made to your child's school breakfast and/or lunch at no extra charge with the proper documentation from a licensed medical professional.

  • The resource below provides some additional information on what is needed for the accommodations as well as a blank copy of our medical documentation form. This form is not mandatory, a note signed by the doctor is sufficient as long as it has all of the details needed to make the accommodations.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation.
